How to become a patient

Medical Appointments:

  1. Select the clinic you wish to attend and call the clinic to make an appointment. Clinics Page
  2. Arrive to your first appointment at least 15 minutes ahead of time to complete your patient intake form.  Bring with you:
  • A copy of your current tax return
  • A copy of a pay stub for the last month for all working adults in the household
  • SSI/Disability proof of benefits
  • Child support

Why we need this information:  As a Federally Qualified Health Center, we charge for services based on household income.  Even if you have insurance (Medicaid, Medicare or private insurance), some policies may not pay for every visit or service.  Therefore, we would need to know what to charge you based on your household income.  This is called a “sliding fee.”  Simply download, print and complete the MEDICAL Form (PDF)  and bring your completed form with you along with any supporting documents listed above.

Walk-ins are welcome.  However, to avoid a potentially long wait around scheduled appointments, you are encouraged to call ahead for the next available time slot.

Dental Appointments:

Since our dental clinics are usually booked weeks ahead of time, you must make an appointment to see us.  Here are the steps you need to take:

  1. Select the dental clinic at which you wish to make an appointment. Clinics Page
  2. Download and print a DENTAL Form (PDF) or stop by one of our dental clinics to receive a form.
  3. Complete and submit the form to the dental clinic (along with the same supporting documents listed for making a Medical appointment above) where you wish to make the appointment.  Why?  Because we need to inform you of your anticipated charges at the time of your appointment based on the insurance coverage you have (or lack of insurance.)
