For MEDICAL services, we accept:

• No insurance?   Payments on a sliding scale for those who do not have insurance. Must bring proof of household income (food stamp letters, pay stubs, tax forms, prior year returns, letters from employers.)

For DENTAL services, we accept:

ATTENTION DENTAL PATIENTS ON MEDICAID:

In the next few months, all children receiving dental services through Florida Medicaid will be assigned to an HMO plan. You have a choice -- DentaQuest or MCNA. Community Health Center (aka PanCare of Florida, Inc.).  Our dental clinics in Bay and Walton Counties accept DentaQuest and MCNA.  For more information please call our Bay County dental clinic at 850-767-3350 or our Walton County dental clinic at 850-880-6568.
